User experiences of eHealth services for the treatment of mental health conditions

Abstract

The use of eHealth services increased rapidly during the COVID-19 pandemic, and they have subsequently become part of the healthcare service offering. This is especially true for mental health problems, as they do not require physical examinations. It is important to know how users experience these services, as this allows the services to be improved. This thesis was a case study that investigated the user experiences of multiple eHealth services for treating mental health problems in Northern Karelia. The main goal of this thesis was to discover what benefits and challenges users experienced related to each eHealth service. In addition, this study sought to assess how the use of eHealth services affected the relationship between service users and professionals. The study data used data from two sources: the open-answer comments of a user survey conducted in fall 2022 and data from five semi-structured interviews with mental health professionals conducted in fall 2023. Based on the collected data, the eHealth services were generally perceived to work well by users, but there were also challenges associated with them. Both users and professionals found remote appointments to be convenient, as they were flexible and eliminated the need to travel to appointments. Remote care also had its challenges: with phone appointments, challenges were mostly related to interpersonal problems and problems in contacting professionals. With video appointments, challenges were mainly related to technical issues or connectivity problems. Some users would have preferred in-person care over remote care, but felt it was difficult to access. MyKanta was seen by users and professionals to be a beneficial service that could increase trust by allowing users to read what had been written about them and check the information for errors. However, users sometimes found errors in their information, and information was occasionally hard to find in MyKanta. Based on the results of the thesis, user experience could be improved if steps were taken to address the challenges of each eHealth service. In addition, users may benefit from being able to choose whether to use eHealth services or in-person services.

Terveydenhuollon etäpalveluiden käyttö on lisääntynyt merkittävästi viime vuosina, ja etäpalvelut ovatkin vakiintuneet osaksi terveydenhuollon palveluvalikoimaa. Tämä pätee erityisesti mielenterveys- ja päihdeongelmien hoitoon, sillä ne eivät välttämättä vaadi paikan päällä tapahtuvia tutkimuksia. On tärkeää selvittää, mitkä seikat terveydenhuollon etäpalveluissa toimivat hyvin ja missä on edelleen haasteita, sillä näin voidaan parantaa asiakkaiden etäpalvelukokemuksia. Tämä tutkimus toteutettiin tapaustutkimuksena Pohjois-Karjalan hyvinvointialueella, jossa selvitettiin asiakkaiden kokemuksia mielenterveys- ja päihdepalveluiden etäpalveluista. Tutkimuksessa pyrittiin lisäksi selvittämään, miten etäpalvelut vaikuttavat ammattilaisen ja asiakkaan väliseen suhteeseen. Tutkimuksen data oli peräisin kahdesta lähteestä: kyselytutkimuksesta, joka toteutettiin mielenterveys- ja päihdepalveluiden asiakkaille sekä viidestä puolistrukturoidusta haastattelusta mielenterveys- ja päihdepalveluiden ammattilaisten kanssa. Sekä asiakaskyselyn että ammattilaishaastattelujen perusteella voidaan todeta, että etäpalvelut toimivat pääosin hyvin, mutta niiden käyttöön liittyy myös haasteita. Asiakkaat ja ammattilaiset pitivät etävastaanottoja kätevinä, sillä ne olivat joustavia ja poistivat tarpeen matkustaa vastaanotolle. Videovastaanottojen pääasialliset haasteet liittyivät teknisiin ongelmiin ja yhteysongelmiin. Puhelinvastaanottojen haasteet liittyivät usein vuorovaikutuksen laatuun sekä ongelmiin saada ammattilaisiin yhteyttä. Jotkut asiakkaat olisivat myös halunneet käydä lähivastaanotolla, mutta kokivat ettei niitä ollut riittävästi saatavilla. OmaKannassa asiakkaat pitivät siitä, että palvelu mahdollisti heille omien terveystietojensa katsomisen, mutta palvelusta löytyi kuitenkin virheitä, ja tietoja oli toisinaan vaikea löytää. Asiakaskokemuksia voitaisiin mahdollisesti parantaa puuttumalla kunkin etäpalvelun haasteisiin ja mahdollistamalla asiakkaille valinnanvapaus etäpalveluiden ja lähipalveluiden välillä.
