Abstract

Poor usability of healthcare IT systems not only lowers their adoption rate, but can have even more severe consequences; it can affect the safety of patients and put their lives at risk. To increase the adoption rate and the safety of the systems, confidence on the systems usability should acquired even before the system is implemented, during the procurement. However, thus far there has not yet been a body of extensive scientific literature addressing the topic. The research goals of this thesis are to 1) map out how usability can be included in the procurement processes of healthcare IT systems and 2) compile best practices into practical framework for procuring usable healthcare IT systems. To meet these goals two approaches were applied literature review and interviews from recent procurements of healthcare IT systems. In the first phase of the study a scoping review was conducted and a preliminary framework was created based on the literature review to identify how the framework model was relating to the current procurement process, in the second phase of the study it was used as a bases for interviews of three cases of recent healthcare IT system procurement projects. The framework was then updated based on the results from the interviews of the three cases, and the refined framework is presented as a conclusion. The framework consists of five phases of procurement and what usability activities should be carried out in each phase. The five phases of procurement are defining strategic goals, creating the requirements, system evaluation, system selection and implementation and training. Especially the need for of creating verifiable and comprehensive usability requirements in the requirement creation phase and testing the usability of the systems in the system evaluation phase with high-fidelity usability testing that includes actual users performing realistic tasks in a realistic context was highlighted.

Terveydenhuollon tietojärjestelmien puutteellisella käytettävyydellä voi olla vakavia seuraamuksia: se vähentää järjestelmän käyttöönottoastetta, vie terveydenhuollon ammattilaisten huomion pois hoitotyöstä ja pahimmillaan jopa asettaa potilaiden turvallisuuden vaaraan. Käyttöönottoasteen ja järjestelmien turvallisuuden parantamiseksi tulisi järjestelmän käytettävyydestä varmistua jo ennen tuotteen käyttöönottoa, hankinnan aikana. Käytettävyyden sisällyttämistä terveydenhuollon tietojärjestelmien hankintaprosessiin ei ole kuitenkaan aiemmin laajalti tutkittu. Tämän tutkimuksen tarkoituksena on 1) kartoittaa, miten käytettävyys voidaan ottaa osaksi terveydenhuollon tietojärjestelmien hankintaprosessia sekä 2) koota käytännön viitekehys käytettävien terveydenhuollonjärjestelmien hankintaan. Tutkimusmenetelminä käytettiin kirjallisuuskatsausta sekä haastatteluja viimeaikaisista terveydenhuollon tietojärjestelmien hankintaprosesseista. Kirjallisuuskatsauksen perusteella luotiin ensin alustava viitekehys. Alustavan viitekehyksen pohjalta suoritettiin haastatteluja kolmesta viimeaikeisesta terveydenhuollon tietojärjestelmähankintaprosessista. Haastattelujen perusteella viitekehystä kehiteltiin edelleen ja paranneltu viitekehys esitetään tämän työn tuloksena. Viitekehys koostuu viidestä hankinnan vaiheesta, sekä kuhunkin vaiheeseen liittyvistä käytettävyystoiminnoista. Nämä viisi vaihetta ovat: Strategisten tavoitteiden asettaminen, vaatimusmäärittely, järjestelmien arviointi, järjestelmän valinta sekä käyttöönotto ja koulutus. Erityisesti korostetaan todennettavien käytettävyysvaatimusten asettamista vaatimusmäärittelyvaiheessa sekä järjestelmien arviointivaiheessa perusteellisen käytettävyystestauksen suorittamista todellisten käyttäjien kanssa mahdollisimman todenmukaisia työtehtäviä ja mahdollisimman realistista kontekstia käyttäen.
