Abstract

The topic of the master’s thesis is about type 2 diabetes patients and their guidance in the beginning of the self-care process. The selection and relevance of this topic is based on the needs of the ageing population, which is one of the rising demographic trends: “Globally, the number of older persons (aged 60 years or over) is expected to more than double, from 841 million people in 2013 to more than 2 billion in 2050.” (World Population Ageing, 2013. United Nations). There will be more seniors needing health care services in the future and diabetes treatment is one of those. Unfortunately, this means that there will not be sufficient resources (time, money, human resources) for a face-to-face communication between the patient and the health care providers. Therefore, the patients have to take more responsibility and control over their own health and the role of self-monitoring and self-medication will grow. The quicker the diabetes patients learn how to manage their own blood glucose levels, the quicker their quality of life will improve. That is not as easy as it sounds, since there are several steps that the patients have to go through in order to get their levels in balance. The focus of the thesis was to study different methods for improving the learning experience of the patients. For this purpose it was important to get an insight to both the health care professionals´ and the type 2 diabetes patients` world. This was done through literature review, which gave an overview of the diabetes as a disease. The current status of the diabetes care and treatment methods were defined after running interviews with nurses, doctors, patients and other professionals working with diabetes related issues. The interviews confirmed my original hypothesis about how incoherent the current instructions are for type 2 diabetes patients. It was also evident, that the instructions and how those were understood played a significant role in the quality and success of care. Therefore, pedagogical theories related to adult learners and their motivation to learn was studied such as Bloom´s taxonomy, which helps to define the level of learning. Next to the interviews, observations and questionnaires, I have used in my design process an affinity diagram, card-sorting method with the patients and created personas out of the findings. As a result of these, it was possible to organize the information flow that the patients received in the beginning of the self-care. The outcome of the thesis is a four-week learning program, which helps the type 2 diabetes patients to get familiar with the equipment, medication routine and tools for managing their diabetes better. The essential parts of this learning program are a starter kit, which is handed out at the clinic, and the weekly follow-up letters, which are sent to the patients home. The concept was prototyped and the feedback was collected from the senior patients with diabetes. The conclusion of the thesis stays partly on a concept and prototype level, but some parts of it will be utilized in Mendor Ltd´s, the collaboration partner´s, product development.

Pro gradu-tutkielman aiheena on kakkostyypin diabeetikkojen omahoidon ohjauksen parantaminen. Aiheen valinta ja ajankohtaisuus perustuu väestön ikääntymiseen, joka on yksi kasvavista trendeistä väestönkehityksessä:“ Maailmanlaajuisesti, ikääntyneiden henkilöiden (60 vuotta tai yli) määrän odotetaan kaksinkertaistuvan 841 miljoonasta ihmisestä vuonna 2013 jopa yli 2000 miljoonaan vuoteen 2050 mennessä.” (WHO, 2013. YK.) Tulevaisuudessa terveydenhoitoa tarvitsevia eläkeläisiä on siis huomattavasti nykyistä enemmän. Diabeteksen hoito kuuluu myös tuohon kasvavaan osuuteen. Valitettavasti tämä tarkoittaa sitä, että resurssit kuten aika, raha ja henkilöstö eivät riitä potilaiden ja terveydenhoidon ammattilaisten välisiin tapaamisiin kasvokkain. Siksi potilaiden on otettava enemmän vastuuta omasta terveydestään ja kontrolloitava sitä parhaansa mukaan. Niinpä omavalvonnan ja itsehoidon rooli tule kasvamaan potilaiden paremman elämänlaadun takaamiseksi. Mitä nopeammin diabetes potilaat oppivat hallitsemaan omaa verensokeriaan, sitä nopeammin heidän elämänlaatunsa paranee. Se ei ole niin helppoa miltä se kuulostaa, sillä potilaat joutuvat käymään läpi useita eri vaiheita saadakseen verensokerinsa tasapainoon. Opinnäytetyön tavoitteena on selvittää, miten eri menetelmiä käyttämällä potilaiden oppimiskokemusta voidaan kehittää. Tätä tarkoitusta varten oli tärkeää saada tietoa sekä terveydenhuollon ammattilaisilta että kakkostyypin diabeetikoiden maailmasta. Kirjallisuuskatsaus antoi yleiskuvan diabeteksesta sairautena. Diabeteksen hoidon ja käytettyjen menetelmien nykytilanne selvitettiin tekemällä haastatteluja sairaanhoitajien, lääkärien, diabetespotilaiden ja muiden diabeteksen parissa työskentelevien ammattilaisten kanssa. Haastattelut vahvistivat alkuperäisen väitteen siitä, miten epäyhtenäiset ohjeet tyypin 2 diabetespotilailla nykyisin on. Oli myös selvää, että ohjeilla ja niiden oikein ymmärtämisellä oli merkittävä rooli hoidon laadun ja menestymisen kannalta. Aikuisiän oppijoihin ja heidän motivaatioonsa liittyviä pedagogisia teorioita tutkittiin kuten Bloomin taksonomiaa, joka auttaa määrittelemään osaamisen tason. Haastatteluiden, havaintojen ja kyselylomakkeiden ohella, suunnitteluprosessissa käytettiin affiniteettikaaviota, korttien lajittelu-menetelmää ja käyttäjäpersoonan luomista saatujen havaintojen perusteella. Näiden tuloksena oli mahdollista lajitella se kaikki se tieto, minkä potilaat ottivat vastaan heidän omahoitonsa alussa. Gradututkielman tuotoksena on neljän viikon oppimisohjelma, joka auttaa kakkostyypin diabeetikkoja tutustumaan saamiinsa hoitovälineisiin, lääkitysrutiineihin sekä käytännön työkaluihin joilla hallita diabetesta paremmin. Tuon oppimisohjelman olennaisin osa oli aloituspakkaus, joka jaettaisiin klinikalla uusille potilaille ja viikoittaiset kotiin kuljetetut hoidon seurantakirjeet. Konseptista tehtyjen prototyyppien avulla oli mahdollista kerätä palautetta seniori-ikäisiltä diabetespotilailta. Opinnäytetyön lopputuotos pysyy osittain konsepti- ja prototyyppi tasolla, mutta joitain osia siitä tullaan hyödyntämään yhteistyökumppanin Mendor Oy:n tuotekehityksessä.
