Abstract

This study examines the costs of organizing social and healthcare services in Finland's wellbeing service counties. The aim of the study is to define the concept of organizing costs, estimate their indicative scale, and the current state of the cost calculation models. The research was conducted using a mixed-methods approach, combining qualitative and quantitative data. The qualitative data consisted of interviews with representatives of the wellbeing service counties and experts, while the quantitative data was derived from the calculation models of the counties and the results they produced. As a result of the study, a definition of organizing costs is presented, based on the legal obligations of the wellbeing service counties, which cannot be applied to private service providers. Additionally, an indicative scale is provided for the costs of round-the-clock care for the elderly and low volume service group. The study found that there are still significant differences in the calculation of organizing costs across regions, making it difficult to compare results and conduct consistent evaluations.

Tässä tutkimuksessa tarkastellaan sosiaali- ja terveydenhuollon järjestämisen kustannuksia Suomen hyvinvointialueilla. Tutkimuksen tavoitteena on määritellä järjestämisen kustannusten käsite, arvioida niiden indikatiivista kokoluokkaa sekä laskennan nykytilaa hyvinvointialueilla. Tutkimus toteutettiin monimenetelmällisesti yhdistäen laadullista ja määrällistä aineistoa. Laadullinen aineisto muodostui hyvinvointialueiden edustajien sekä asiantuntijoiden haastatteluista ja määrällinen aineisto hyvinvointialueiden laskentamalleista sekä niiden tuottamista tuloksista. Tutkimuksen tuloksena esitetään järjestämisen kustannusten määritelmä, joka pohjautuu hyvinvointialueiden lakisääteisiin velvoitteisiin rajaten pois ne, jotka koskevat myös yksityisiä palveluntuottajia. Lisäksi tutkimuksessa esitetään indikatiivinen kokoluokka ikääntyneiden ympärivuorokautisen hoivan sekä pienemmän volyymin palveluiden ryhmille. Tutkimuksessa havaittiin, että järjestämisen kustannusten laskennassa on edelleen merkittäviä eroja alueiden välillä, mikä vaikeuttaa tulosten vertailtavuutta ja johdonmukaisen arvioinnin tekemistä.

I denna studie undersöks kostnaderna för ordnandet av social- och hälsovårdstjänster inom Finlands välfärdsområden. Syftet med studien är att definiera begreppet kostnader för ordnandet, uppskatta deras indikativa storleksordning samt kostnadsberäkningsmodellernas nuläge. Studien genomfördes med en mångmetodisk ansats där kvalitativ och kvantitativ data kombinerades. Den kvalitativa datan samlades in genom intervjuer med representanter från välfärdsområdena och experter, medan den kvantitativa datan bestod av välfärdsområdenas beräkningsmodeller och de resultat de producerat. Studien presenterar en definition av begreppet kostnader för ordnandet, vilken baseras på välfärdsområdenas lagstadgade skyldigheter och utesluter de skyldigheter som även gäller privata tjänsteproducenter. Dessutom presenteras en indikativ storleksordning för äldreomsorg och tjänster med mindre volymer. Studien visar att det fortfarande finns betydande skillnader mellan områdena när det gäller beräkningen av kostnaderna för ordnandet, vilket försvårar jämförbarheten av resultaten och försvårar en konsekvent bedömning.
