Using preoperative patient data for improved process management in planned surgical healthcare

Abstract

Healthcare operations around developed nations face the pressure of more demand and limited resources. The productivity of surgical services can be developed with the same mechanisms as used in other industries. IT systems can be used for processing data and producing information which could be used for better process control. The hospital district of Helsinki and Uusimaa (HUS) had developed their hip replacement patient process. The development was done by implementing an IT system enabling patients to deliver preoperative information to the hospital, but HUS had no data if the development had been successful. This thesis had the aim of answering the main research question of "What possibilities collecting preoperative patient data opens in elective surgical healthcare?" This was studied by exploring the processes in the hospital, by collecting and analyzing patient time usage data and by comparing findings to previous research and to other hospitals. A theory framework was constructed to discuss the effects of the context of a hospital, interventions themselves, mechanisms that would affect the service production and functionality of the desired outcome. Results showed that the interventions didn't have the desired outcome of shorter lead-times and fewer anesthetist visits. This was due to keeping the old processes and not reviewing the data patients had delivered before their arrival to the hospital. Literature review, comparison to different hospitals and interviews proved that if the implementation was successful, over half of the current anesthetist visits could be removed. In addition to this, new information could be used for employee allocation at a level that was previously impossible. Also, the information could be used for creating groups of patients that would be consulted by physiotherapists instead of the current individual visits.

Terveydenhuollon palvelutuotannon haasteena on lisääntyvä kysyntä ja rajalliset resurssit. Kirurgisten palveluiden tuottavuutta voidaan kehittää samoilla menetelmillä, joita on käytetty muilla toimialoilla. Tietojärjestelmien avulla voidaan tehostaa datan käsittelyä ja tuottaa informaatiota, jota voi käyttää parempaan palveluprosessin hallintaan. Helsingin ja Uudenmaan sairaanhoitopiiri (HUS) oli kehittänyt lonkkaleikkausten potilasprosessia ottamalla käyttöön tietojärjestelmän, jonka avulla potilaat pystyivät toimittamaan tietoa sairaalaan ennen leikkausta edeltävää käyntiä. HUS:lla ei kuitenkaan ollut dataa siitä, oliko tietojärjestelmän käyttöönotto tuottanut tuloksia. Tämän diplomityön tavoitteena oli vastata pääasialliseen tutkimuskysymykseen: "Mitä mahdollisuuksia potilasesitiedon kerääminen avaa kirurgisen terveydenhuollon kehittämisessä?" Tähän vastattiin tutkimalla sairaalan prosesseja, keräämällä ja analysoimalla potilaan ajankäyttöön liittyvää dataa ja vertaamalla havaintoja aiempaan tutkimukseen ja muiden sairaaloiden käytäntöihin. Työn teoriaviitekehys huomioi sairaalaympäristön, käyttöönoton toteutuksen, tuotantoon vaikuttavien mekanismien vaikutuksen ja tavoiteltavien tulosten tarkoituksenmukaisuuden. Tulokset osoittivat, että järjestelmän käyttöönotolla ei saavutettu tavoiteltua lyhyempää läpimenoaikaa eikä vähemmän anestesialääkärikäyntejä. Tämä johtui siitä, että sairaala jatkoi vanhojen prosessien käyttämistä ja potilaan toimittamia tietoja ei tarkasteltu ennen tämän saapumista sairaalaan. Kirjallisuuskatsaus, vertailu muihin sairaaloihin ja haastattelut osoittivat sen, että mikäli käyttöönotto olisi onnistunut, jopa yli puolet nykyisistä anestesialääkärikäynneistä voisi jättää pois. Tämän lisäksi informaatiota voisi hyödyntää työvuorojen suunnittelussa sekä potilasryhmien muodostamisessa fysioterapeutin tapaamista varten nykyisten yksittäisten tapaamisten sijaan.
