Abstract
Pharmacokinetic/pharmacodynamic (PK/PD) models can be used during a surgical operation to estimate patient level of consciousness. This thesis researches how the accuracy of these models could be improved by using the values measured from the patient. The approach is to normalize the PK/PD model result, so that it is same as value measured by a depth of anesthesia monitor. In this thesis existing clinical data is used to develop and test a software prototype.
The conclusion of this thesis is that the accuracy of a PK/PD model can be improved by using a combination of non-linear normalization and time delay estimation. In real life applications this enables new features such as a patient dependent scale for PK/PD model results or prediction of the values measured by a depth of anesthesia monitor.
While this thesis demonstrates the technical feasibility of the approach, it did not prove the practical usability or clinical value and these have to be assessed in further studies.
Farmakokineettis-dynaamisia (PK/PD) malleja voidaan käyttää kirurgisen toimenpiteen aikana arvioimaan potilaan tajunnan tasoa. Tämä opinnäytetyö tutkii miten näiden mallien tarkkuutta voitaisiin parantaa käyttämällä potilaasta mitattuja arvoja. Lähestymistapana on normalisoida PK/PD-mallin tulos, niin että se on sama kuin anestesian syvyyden monitoroinnin mittaama arvo. Tässä työssä prototyyppiohjelma kehitetään ja testataan käyttäen olemassa olevia kliinisiä tietoja.
Tutkimuksessa havaittiin, että PK/PD-mallin tarkkuutta voidaan parantaa käyttämällä epälineaarisen normalisoinnin ja aikaviipeen arvioinnin yhdistelmää. Käytännön sovelluksissa tämä mahdollistaa uusia ominaisuuksia, kuten potilaasta riippuvaisen asteikon PK/PD-mallin tuloksille tai anestesian syvyyden monitoroinin mittaamien arvojen ennustamisen.
Vaikka tämä tutkielma osoittaa lähestymistavan teknisen toteutettavuuden, se ei osoittanut sen käytännön käytettävyyttä tai kliinistä arvoa vaan nämä on arvioitava jatkotutkimuksissa.
