Extraction of relevant electroencephalogram features for detection of epileptic seizures in the intensive care unit

Abstract

Epileptic seizures are disturbances in the normal electrical activity of the brain. They are especially common in the intensive care units (ICUs) where seizures are usually caused by the primary condition that the patients have. Depending on the patient population approximately 8 - 68 % of the patients in ICU suffer from epileptic seizures. Most of these seizures also lack clear clinical signs and therefore monitoring of the brain activity using electroencephalography (EEG) is required in order to detect them.

Continuous EEG monitoring generates a huge amount of raw data that must he somehow pre-screened before it can be efficiently utilized in the treatment of the ICU patients. Methods that automatically detect seizures from EEG recordings have been developed since 1970s, but all of them have been developed either for patients in epileptic monitoring units (EMUs) or for neonatal patients. The EEG signs of seizures in the ICU patients are known to differ somewhat from those of these two patient groups. Therefore there is need for new methods of computerized seizure detection.

In this study an extensive review of the current seizure detection methods is given, and a set of quantitative EEG features is evaluated for its usefulness in the detection of ICU seizures. The obtained results offer good insight on where the future efforts in the development of the seizure detection methods should be focused.

Epileptiset kohtaukset ovat aivojen normaalin sähköisen toiminnan häiriöitä. Ne ovat erityisen yleisiä teho-osastolla, jossa ne ovat usein potilaiden alkuperäisten vammojen tai sairauksien aiheuttamia. Riippuen potilasryhmästä arviolta 8 - 68 % teho-osastopotilaista kärsii epileptisistä kohtauksista. Suurimmassa osassa näitä kohtauksia ulkoiset oireet ovat vaikeasti havaittavia, joten aivojen toiminnan monitorointia elektroenkefalografian (EEC) avulla tarvitaan niiden havaitsemiseksi. Jatkuva EEG-monitorointi tuottaa valtavan määrän raakadataa, jota on esikarsittava ennen kuin se voidaan ottaa tehokkaasti käyttöön potilaiden hoidossa teho-osastolla. Menetelmiä epileptisten kohtausten automaattiseen tunnistukseen EEG-nauhoituksista on kehitetty 1970-luvulta lähtien. mutta kaikki niistä on kehitetty käytettäväksi epilepsiamonitorointiyksiköissä tai vastasyntyneiden hoidossa. Teho-osastopotilailla kohtausten ilmenemät EEG-nauhoituksissa poikkeavat hieman mainittujen potilasryhmien vastaavista. Tämä aiheuttaa tarpeen uusien tietokonepohjaisten menetelmien kehittämiselle kohtausten tunnistukseen.

Tämä työ sisältää laajan katsauksen aikaisemmin kehitettyihin kohtauksen tunnistusmenetelmiin. Lisäksi joukko kvantitatiivisia EEC-piirteitä on evaluoitu, tarkoituksena löytää hyödyllisiä piirteitä käytettäväksi tietokonepohjaisten menetelmien jatkokehityksessä. Saadut tulokset antavat hyviä suuntaviivoja tulevaa kohtauksentunnistusmenetelmien parissa tehtävää tutkimustyötä varten.
