Sote-kiinteistöinvestointien arviointikriteerit ja -menetelmät

Abstract

Sote-kiinteistöinvestointien arviointiin ja niihin liittyvien sairaansijojen ja ikääntyneiden pitkäaikaispaikkojen määrän määrittelyyn ei ole olemassa yksiselitteistä vastausta, vaan päätöksiin vaikuttavat lukuisat tekijät. Hankkeen keskeinen havainto oli, että sairaaloiden hoitojaksot ovat vähentyneet ja niiden kestot lyhentyneet. Asiantuntijat uskovat, että kehitys jatkuu tulevaisuudessa. Ikääntyneiden pitkäaikaispaikkojen määrä on kasvanut, mutta hitaammin kuin ikääntyneiden määrä. Tarpeiden jatkuvasti muuttuessa kiinteistöjen muunto- ja käyttöjoustavuus ovat avainasemassa. Teknologisen kehityksen, digitalisaation ja hoitomenetelmien kehittymisen myötä lisääntyvät etä- ja liikkuvat palvelut voivat vähentää tarvetta rakentaa fyysisiä tiloja palveluiden järjestämiseksi. Henkilöstön saatavuus on suuri haaste ja se vaikuttaa mahdollisuuksiin toteuttaa kotiin vietäviä palveluja. Hankkeessa tuotettiin viitekehys, johon on koottu näkökulmia, joita tulisi huomioida kiinteistöinvestointeja arvioitaessa. Viitekehys jakautuu kahteen osa-alueeseen. Ensimmäisessä osa-alueessa kesitytään investoinnin tarpeellisuuden ja tarvittavan koon arviointiin. Arvioinnin tueksi tuotettiin Excel-pohjainen ennustemalli alueittain vuoteen 2040 asti. Mallin pohjalta rakennettiin kolme skenaariota tulevaisuuden paikkamäärätarpeelle. Toisessa osa-alueessa arvioidaan investointisuunnitelmaa eri näkökulmista.

The assessment of investments in social and health care facilities and the associated number of hospital and assisted living or nursing home beds is influenced by a number of different factors. The key finding of the project was that the number of inpatient episodes in hospitals has decreased and the length of stay shortened. Experts expect this development to continue. The number of assisted living or nursing home beds for the elderly has increased, but at a slower rate than the number of elderly. As needs continue to change, flexibility in the use and conversion of facilities is key. With the advancement of technology, digitalisation and the development of treatment methods, increasing remote and mobile services are reducing the need to build physical facilities to provide services. The availability of staff is a major challenge and affects the ability to implement take-home services. The project produced a framwork that brings together perspectives that should be considered when evaluating investments in facilites. The framework is divided into two parts. The first part focuses on assessing the need for and size of the investment. To support the assessment, an Excel-based forecast until 2040 model was produced. Based on the model, three scenarios were built for future needs. The second part includes numerous qualitative aspects regarding the need for the incestment as well as the quality of the investment plan.
