Telehealth and Multimodal Clinical Pathways in Occupational Health Care: A Supply Chain Management Perspective

Abstract

Despite its acknowledged benefits, studies frequently report the failure of adopting and normalising telehealth as part of routine health care delivery. The coronavirus disease 2019 pandemic has forced health care providers to swiftly adopt new remote modalities of health care, enabling the empirical research of telehealth development. Quantitative data regarding telehealth service provision, and results from specialist interviews in a private health care company were analysed to assess, how multimodal clinical pathways should be managed as the proportion of telehealth increases.

The objective of the research was to bring light to three topics: 1) How has the COVID-19 pandemic impacted the adoption of telehealth in occupational health care, 2) which services can be delivered remotely in the long run, and 3) which operational and system level factors enable scalability of multimodal clinical pathways? The results imply, that due to its preventive nature, occupational health care has high aptness for telehealth. However, as the responsibility of the patient in ensuring the continuity of clinical pathways increases in telehealth, so does the risk of the care process ending at agent handovers. Additionally, a granular service channel mapping was formulated based on the analysis results. The results show, that by focusing on clear service-channel differentiation, health care providers can increase their system capacity through decreased variance in service durations. Lastly, a hierarchy for telehealth’s long-term integration requirements is provided from the perspectives of customer, specialist, process and technology.

By bringing a supply chain management perspective to telehealth and focusing on clinical pathways and care delivery, the research aims at providing novel approaches for both academia and health care decision makers for studying the efficacy and aptness of telehealth as a continuum of care rather than individual interventions and health care services.

Todetuista hyödyistään huolimatta terveydenhuoltoalan tutkimus raportoi toistuvasti etäpalveluiden käyttöönoton sekä normalisoinnin epäonnistumisesta. COVID-19-pandemia on pakottanut terveydenhuollon toimijat omaksumaan uusia palvelukanavia nopeasti mahdollistaen etäpalveluiden empiirisen tutkimuksen. Tässä tutkimuksessa arvioidaan, kuinka monikanavaisia hoitopolkuja tulee johtaa etäpalveluiden osuuden kasvaessa. Tutkimus hyödyntää kvantitatiivista dataa etäpalvelutarjonnasta sekä tuloksia ammattilaishaastatteluista yksityisessä terveydenhuoltoalan yrityksessä.

Tutkimus pyrkii vastaamaan kolmeen kysymykseen: 1) Kuinka COVID-19-pandemia on vaikuttanut etäpalveluiden käyttöönottoon työterveyden piirissä, 2) mitkä palvelut soveltuvat etäkanaviin pitkällä aikavälillä, sekä 3) mitkä operatiiviset ja systeemitason tekijät mahdollistavat monikanavaisten hoitopolkujen skaalautuvuuden? Tuloksista selviää, että ennaltaehkäisevän luonteensa vuoksi työterveydenhuolto soveltuu hyvin etäkanaviin. Toisaalta potilaan vastuu hoitopolun jatkuvuudesta kasvaa etäkanavissa, mikä kasvattaa hoitopolun katkeamisen riskiä agenttivaihdoksissa. Lisäksi tulosten pohjalta koottiin yksityiskohtainen palvelu-kanava-kartoitus. Palvelujen selkeä kanavakohtainen erittely mahdollistaa terveydenhuoltojärjestelmän kapasiteetin lisäämisen vähentämällä palvelutapahtumien keston varianssia. Tutkimus valottaa myös etäpalvelujen integraatioon vaadittavia tekijöitä asiakkaan, ammattilaisen, prosessin sekä teknologian näkökulmasta.

Tuomalla toimitusketjun hallinnan näkökulman etäpalveluihin ja keskittymällä hoitopolkuihin hoidon tuotannossa tutkimus pyrkii tarjoamaan uusia lähestymistapoja sekä akateemiseen tutkimukseen että terveydenhuollon päätöksentekijöille. Etäpalveluiden vaikuttavuutta sekä sopivuutta tulee tutkia hoidon jatkumona yksittäisten hoitotapahtumien sijaan.
